I went to the gum clinic years ago for a check up (i didnt have anything) well i want to go again as i have a new partner-thing is im really embarrassed about going back again-can i just give them a false address so i have new notes?

Well they are
annonymus and confidential, they dont judge and most of the time help out a lot. i dont understand
why you would wanna
give out a fake address. but if you must, of course you can, unless the persons name is already filed.....
